The Certified Diabetes Educator (CDE) works within the Chronic Condition Management (CCM) program as a direct care provider. The CDE is responsible for assessment, planning, and implementation of a customized educational plan/program for Chronic Condition Program participants. The CCM program has a strong focus on patients with diabetes, and its co-morbidities, but also includes other chronic conditions based on population need.  The CDE will provide skilled clinical care for the patient through consultations and referrals to the multidisciplinary team.  The CDE will actively reach out to bring new patients with chronic conditions into the CCM program, and to provide continued support and education for current CCM participants.
  • Works with the patient and the multidisciplinary care team, integrating patient assessment findings and health history information to develop a comprehensive and individualized self-management care plan for patients enrolled in the CCM program
  • Assesses  effectiveness of clinical interventions through evaluation of health data and measurement  of learning objectives attainment
  • Monitors each patients progress and completion of program requirements and conducts follow-up as needed to ensure completion of program requirements
  • Documents assessment data, education provided, clinical interventions and plan including recommendations and goals for each participant in the  electronic medical record. Utilizes best practice documentation guidelines
  • Utilizes a strong team approach involving the multidisciplinary team and patient to deliver high quality coordinated care
  • Practices within legal and personal scope of practice and actively refers patients to other care givers when patients needs exceed that scope
  • Actively supports all requirements related to AAAHC accreditation
  • Responsible for continuous learning and the application of current best practice to all patient care activities
  • Reviews and updates education content and materials annually
  • Willingly expands skill set to include new chronic condition diagnoses per population need (as scope allows)
Appropriate education and/or experience may be substituted on an equivalent basis


  • Associate’s Degree from an accredited college or university
  • Candidates with a RN background - Bachelor’s in Nursing (RN), preferred
  • Candidates with a RDN background - Graduate of an ACEND-accredited dietetic program, preferred


  • If RN candidate has associate degree must also have 5 years current experience working as a CDE; for all other candidates 0-2 years of experience in direct care of patients with diabetes (2 years or more preferred)
  • Additional experience in any of the following is preferred: management of patients with chronic conditions, including but not limited to COPD, asthma, and/or hypertension
  • Insulin pump experience preferred 
Certificates, Licenses, Registrations: 
  • Certified Diabetes Educator certification preferred, or willing to obtain within 1 year (special circumstances may allow for a 2-year completion period)
  • Current and valid RN or RDN license for assigned location(s) of care
  • Current CPR (BLS) certification required
Knowledge, Skills & Abilities: 
  • Strong organizational and interpersonal skills and able to complete work assignments accurately and in a  
    timely manner
  • Able to lead and effectively manage individual and group education sessions
  • Ability to independently determine appropriate course of action in more complex situations
  • Intermediate computer skills (use of microsoft office suite and email)
  • Electronic Medical Record experience preferred.
  • Ability to work independently, exercise creativity, be attentive to detail, and maintain a positive attitude
  • Ability to effectively work with a multidisciplinary care team
  • Ability to prioritize and manage multiple, and simultaneous responsibilities
  • Ability to effectively communicate, both orally and in writing, across all levels of the care team and    organization
  • Ability to maintain confidentiality of all medical, financial, and legal information
  • Ability to handle difficult situations involving patients, physicians or others in a professional manner
  • Bilingual a plus
