Employee Spotlight: Rachel Rhodes, Condition Management/Take Control, Program Coordinator

A day in the life

In her role, Rachel’s typical day is centered around running and analyzing reports that guide her daily responsibilities, including outreach calls, tracking program compliance, communicating with patients, and scheduling appointments. She begins each day by logging in, reviewing her meetings, opening key applications, and organizing her priorities.

Rachel is actively involved in supporting the TakeControl program, where she helps ensure, patients meet requirements and remain engaged, while also conducting outreach to increase participation. In addition, she is contributing to a newly launched weight management program, gaining valuable experience in building and implementing it from the ground up.

Collaboration is an important part of her work, as she regularly partners with the Condition Management Care Coordinator at the Newport News, VA location, as well as CCM educators and her supervisor, to align workflows and maintain efficiency. She especially values the sense of accomplishment she feels at the end of each day. Rachel also appreciates the company’s support for work-life balance, allowing her to fully disconnect after work and spend meaningful time with her family.

Career growth and development

Rachel began her journey with QuadMed after being encouraged by a former colleague to apply for a clinic nurse position, which she successfully stepped into before advancing into her current role as a Condition Management Care Coordinator. Since joining the company in 2021, she has grown within her role, transitioning early on to better align with her work-life balance. Throughout her time at QuadMed, Rachel has embraced opportunities to learn and take on new challenges, supported by leadership that encourages her growth.

She has utilized professional development resources, including continuing education and hands-on projects, to strengthen her skill set. Rachel advises new employees to stay focused on their goals, remain open to growth, and take full advantage of the opportunities and benefits available while maintaining a healthy work-life balance.

Meaningful and challenging work

Rachel finds her work especially meaningful as it marked her transition from a 13-year clinical nursing career into a non-clinical role, where she has continued to grow and develop new skills. She is particularly proud of her involvement in launching the Weight Management program, where she helped implement multiple phases, train staff, and support patients—an effort that earned her recognition with a four-pillar award.

She navigates challenges such as evolving workflows and new program launches by focusing on understanding processes and asking questions, allowing her to better support her team. Rachel takes pride in making a positive impact by helping patients, supporting colleagues, and improving workflows, drawing on her diverse experiences across clinical and non-clinical roles.

She remains motivated by the company’s strong alignment with her values, a supportive team environment, and the opportunity to make meaningful contributions. Whether mentoring new employees, assisting as an EPIC Superuser, or helping implement new ideas, Rachel values the ability to make a real difference and finds her work both rewarding and fulfilling.
